Job summary

Berlin Health & Rehabilitation Center in Barre, Vermont is seeking a dedicated Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team.

New graduates are welcome to apply. Pay is $42–$55 per hour, with a comprehensive, patient-centered approach to functional independence and safety.

The OT will evaluate residents, develop treatment plans, provide interventions for ADLs, and collaborate with an interdisciplinary team to promote quality care.

Qualifications

• Active and valid Occupational Therapy license in Vermont.
• Graduate of an accredited Occupational Therapy program.
• Experience in a skilled nursing facility or long-term care setting preferred.
• Knowledge of OT treatment practices and interventions.
• Familiar with electronic documentation systems (Net Health or similar).
• Strong clinical and interpersonal abilities.

Responsibilities

• Evaluate residents and patients to determine OT needs, goals.
• Develop individualized treatment plans based on evaluations and orders.
• Provide OT interventions for ADLs, mobility, strength, cognition, balance.
• Create rehabilitation programs to improve independence and safety.
• Supervise OT assistants and ensure regulatory compliance.
• Document evaluations, progress notes, and discharge planning.
