Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based healthcare for local Medicare populations at our neighborhood centers.

The role emphasizes quality of care over volume, with a focus on preventive medicine and positive patient experiences. Board-certified or eligible physicians with MD/DO degrees are encouraged to apply. Full-time schedule Monday–Friday 8:00 AM–5:00 PM with competitive compensation.

Qualifications

• MD or DO degree required.
• Internal Medicine or Family Medicine board certification (or eligible).
• Active, unrestricted State License and DEA license required.
• US work authorization required.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee and administer primary care services.
• Prescribe and manage medications for patients.
• Maintain Patient EMR data via Canopy and Greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Direct and manage the center care team with Practice Managers.
• Other duties as assigned.
