Oak Street Health is seeking a Primary Care Physician to provide equitable, value-based care to local Medicare populations across our neighborhood centers. You will work within large care teams to focus on quality over volume and participate in an outcomes-driven care model.

New graduates are encouraged to apply. A Center Medical Director path may be considered for highly qualified candidates, with a part-time schedule and competitive benefits.

Qualifications

• MD or DO graduate with medical license in good standing.
• Board-certified or board-eligible in Internal Medicine or Family Medicine.
• Eligibility to work in the US and hold a valid DEA license.

Responsabilités

• Assess and diagnose patients at our local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medication.
• Maintain Patient Electronic Medical Record data via canopy and greenway.
• Collaborate with regional and central leadership to meet health quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Other duties as assigned
