Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our growing team in a SNF setting. You will evaluate and treat patients following an individualized plan of care and collaborate with facility staff to ensure high quality care.

Responsibilities include patient evaluations, developing plans of care, treating patients, supervising PTAs, re-evaluating progress, and documenting evaluations and progress notes. Excellent patient service is essential for success in this role.

• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
