Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in SNF settings. You will evaluate and treat patients following an individualized plan of care and collaborate with facility staff to ensure high-quality, patient-centered outcomes.

In this role you will supervise Physical Therapist Assistants, re-evaluate progress, adjust plans of care, and maintain timely, accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.

Responsibilities

• Provide excellent care for patients.
• Deliver excellent customer service to patients and families.
• Collaborate with facility staff to ensure high-quality care.
• Evaluate new patients and develop plans of care with measurable goals.
• Treat patients, address concerns, and explain injuries and progress.
• Supervise Physical Therapist Assistants.
• Re-evaluate patient progress and adjust plans of care as needed.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.

• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
