Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care and collaborate with facility staff to ensure high-quality care for our patients.

You will supervise Physical Therapist Assistants, re-evaluate progress, adjust plans, and document evaluations and daily notes to reflect patient progress. This role focuses on delivering compassionate care and measurable outcomes.

• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes

• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
