Job summary

Enhance Rehabilitation is currently seeking qualified Physical Therapists to join our dynamic team in SNF settings. You will evaluate and treat patients following an individualized plan of care.

This role focuses on delivering high-quality patient care, collaborating with facility staff, and maintaining thorough documentation of evaluations and patient progress. You will educate patients on their injuries, progress, and probable outcomes while supervising Physical Therapist Assistants as needed.

Qualifications

• Qualified Physical Therapists for SNF setting.
• Ability to evaluate and treat patients per individualized plan of care.

• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
