Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care and work closely with facility staff to ensure high-quality outcomes.

You will supervise Physical Therapist Assistants, re-evaluate progress, revise plans of care, and document evaluations, re-evaluations, discharges and daily progress notes in a timely, accurate manner.

Qualifications

• Evaluating new patients and developing plans of care.
• Setting measurable, functional goals.
• Supervising Physical Therapist Assistants.
• Completing timely documentation including evaluations, re-evaluations, discharges and daily progress notes.
