Job summary

Creative Solutions in Healthcare - West is seeking a Full-Time Physical Therapist (PT) to join our in-house rehab team. You will provide person-centered care to help residents improve mobility, strength, and overall quality of life through individualized plans of care.

The ideal candidate holds a Texas PT license, with SNF or long-term care experience preferred, and is proficient with Net Health Electronic Documentation System.

Qualifications

• Active and valid Texas Physical Therapist license
• In good standing with all regulatory agencies and licensing boards
• SNF or long-term care experience preferred
• Current knowledge of physical therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying personalities and needs
• Strong communication, collaboration, and clinical decision-making skills

Responsibilities

• Consult with patients to assess their physical condition and functional limitations
• Evaluate and interpret patient assessments and test results
• Develop individualized treatment plans using a variety of therapy techniques
• Create personalized rehabilitation programs tailored to each patient’s goals
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing
• Ensure all treatments are delivered in accordance with the established plan of care
• Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team
• Provide clinical support and education to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
• Comply with all applicable rules, regulations, and procedures
• Attend required meetings as designated by the Director of Rehab
