Job summary

Autumn Lake at Madison in Madison, CT is seeking a Physical Therapist Assistant (PTA) for a full-time position. The PTA will provide therapy under a licensed PT, help residents with mobility and gait training, and implement individualized treatment plans.

The role requires an active Connecticut PTA license and PTA program graduation, with strong communication and teamwork. SNF rehab experience is a plus, but new grads are welcome to apply, with opportunities for growth within a collaborative

Qualifications

• Active Connecticut Physical Therapist Assistant license required.
• Graduate of an accredited Physical Therapist Assistant program.
• Strong communication and teamwork skills.

Responsibilities

• Provide physical therapy treatments under the direction of a licensed Physical Therapist.
• Assist residents with strengthening, balance, gait training, transfers, endurance, and mobility.
• Carry out individualized treatment plans established by the supervising PT.
• Monitor resident progress and communicate updates to the therapy team.
• Educate residents and caregivers on exercises and safe mobility techniques.
• Complete accurate and timely clinical documentation.
• Collaborate with therapy, nursing, and interdisciplinary team members.
