Autumn Lake at Madison seeks a Physical Therapist Assistant (PTA) to join our rehabilitation team. This full-time role focuses on hands-on care to help residents improve strength, mobility, balance, and independence.

Under the supervision of a licensed PT, you will implement treatment plans, monitor progress, document accurately, and collaborate with therapy, nursing, and interdisciplinary staff. We offer competitive pay and comprehensive benefits, plus opportunities for professional growth.

Formación

• Active Connecticut PTA license required.
• Graduate of an accredited Physical Therapist Assistant program.
• Strong communication and teamwork skills.
• New graduate PTAs are welcome to apply.
• Experience in skilled nursing or rehabilitation preferred but not required.

Responsabilidades

• Provide physical therapy treatments under the direction of a licensed Physical Therapist.
• Assist residents with strengthening, balance, gait training, transfers, endurance, and mobility.
• Carry out individualized treatment plans established by the supervising PT.
• Monitor resident progress and communicate updates to the therapy team.
• Educate residents and caregivers on exercises and safe mobility techniques.
• Complete accurate and timely clinical documentation; collaborate with therapy, nursing, and interdisciplinary team members.
