Job summary

NORTH EAST MEDICAL SERVICES seeks a PACE Escort to provide escort services between participants’ homes, medical appointments, and the PACE center in San Francisco. The role emphasizes safety, comfort, and dignity while assisting with activities of daily living as needed.

You will coordinate with the interdisciplinary team, support participants in-home and at the PACE center, and document changes in condition per care plans. Fluency in English is required; Cantonese/Mandarin is preferred.

Qualifications

• Must complete and pass the North East Medical Services PACE training curriculum upon hire.
• Current BLS and First Aid required
• Ability to communicate effectively.

Responsibilities

• Escort participants between homes, medical appointments, and the PACE center in a safe, timely manner.
• Accompany participants during transport to ensure comfort and safety.
• Assist participants with entering/exiting vehicles and using durable medical equipment.
• Provide basic non-medical support during escort and transport, including reminders and observation.
• Check-in participants and hand off to staff at facilities or the PACE center.
• Maintain confidentiality and respect participant privacy at all times.
