Job summary

Landmark of Plano Rehabilitation and Nursing Center is seeking a dedicated Occupational Therapist (OT) to join our in-house rehab team. You will evaluate residents, design and implement person-centered treatment plans, and collaborate with a multidisciplinary team to maximize ADLs and independence.

Texas-licensed OT with SNF or long-term care experience is preferred. Strong communication and documentation skills are essential to provide high-quality care and education to residents and families.

Qualifications

• Active and valid Texas OT license.
• In good standing with regulatory agencies.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Responsibilities

• Consult with patients to evaluate their physical condition and its impact on daily occupations.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs and IADLs.
• Assess home and living environments and recommend modifications.
• Recommend adaptive equipment and provide training to support independence and safety.
• Ensure treatments are delivered per care plan and adjust as needed.
• Monitor patient response and modify interventions in consultation with the team.
• Provide clinical education and support to patients, family members, and caregivers.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Complete documentation thoroughly and timely in compliance with policies and regulations.
• Attend required meetings as designated by the Director of Rehab.
