Job summary

The Legacy Senior Communities is seeking a licensed Occupational Therapist to provide intermittent home health therapy in Plano, TX. This PRN role serves clients in their homes, following physician orders and collaborating with the care team to achieve independent function.

The OT will evaluate, plan, implement, and document therapy, educate clients and families, and participate in discharge planning. A Texas OT license and related experience are required.

Qualifications

• Graduate of a physical therapy school approved by the AMA/AOTA.
• Current license in the state of Texas as an OT.
• At least one year of institutional experience is required.

Responsibilities

• Perform intermittent skilled occupational therapy visits in clients' homes.
• Develop and update treatment plans in coordination with physicians.
• Educate clients and families on therapy goals and home techniques.
• Document evaluation data, treatments and progress within 24–48 hours.
• Coordinate scheduling with Intake Coordinator and participate in team conferences.
• Assist with discharge planning and complete required summaries.
