Concept Rehab is seeking a Full-Time Occupational Therapist to join our therapy team in the Ingham, MI area. You will evaluate patients, develop and implement individualized care plans, and deliver skilled OT interventions to help patients regain independence in daily activities.

Responsibilities

• Evaluate patients and identify functional goals.
• Develop individualized plans of care.
• Provide skilled occupational therapy interventions.
• Help patients improve independence with everyday activities.
• Track progress and modify treatment based on patient response.
• Communicate with the interdisciplinary care team.
• Participate in care planning and discharge recommendations.
• Complete required clinical documentation.
• Provide guidance and supervision to OTAs as applicable.
• Build positive relationships with patients, families and caregivers.
