Job summary

Concept Rehab, Inc. in Adrian, MI is seeking an Occupational Therapist (OT) to join our rehabilitation team. We are open to full-time or part-time candidates, offering a rewarding role to make a difference in patients' lives.

Youll evaluate patients, develop individualized OT plans, collaborate with a diverse care team, and help patients gain independence and safety through skilled OT services.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Michigan OT licensure or eligibility for licensure.
• Strong clinical, communication, and organizational skills.
• Compassionate and patient-focused approach.
• Ability to work effectively as part of an interdisciplinary team.
• Previous rehabilitation or skilled nursing experience is a plus.

Responsibilities

• Evaluate patients and develop OT treatment plans
• Provide skilled OT services based on each patient’s needs and goals
• Monitor and document patient progress
• Modify treatment plans as appropriate
• Collaborate with PT, SLP, nursing, physicians, and other members of the care team
• Educate patients, families, and caregivers
• Promote patient independence, safety, and quality of life
