Job summary

Brio Living Services is seeking a compassionate LPN for the night shift at Chelsea, MI. This position is part-time (24 hours/week) with 6:30pm–7:00am timing, within the Kresge Nursing & Rehabilitation team.

You will administer medications, monitor residents, mentor nursing assistants, and help deliver high-quality, person-centered care while maintaining safety and accurate records.

Qualifications

• Current Licensed Practical Nurse (LPN) licensure in the State of Michigan.
• Current CPR certification required or willing to obtain upon hire.
• Previous experience in a skilled nursing or long‑term care environment.
• Position requires the ability to lift and move objects weighing up to 35 pounds without assistance.
• Previous experience working with the elderly/geriatric care

Responsibilities

• Deliver Direct Nursing Care – Administer medications, perform treatments, monitor vital signs and wound care.
• Conduct Health Assessments – Evaluate residents and report changes in condition to the clinical team.
• Monitor and Adjust Care Plans – Observe residents for adverse reactions or side effects and update care plans accordingly.
• Coordinate Interdisciplinary Care – Work with teams to enhance residents’ quality of life.
• Supervise and Mentor Staff – Oversee nursing assistants and support staff, providing guidance and mentorship.
• Document Resident Care – Complete daily and monthly records accurately, reflecting care provided and resident responses.
• Manage Admissions and Discharges – Perform admission and discharge procedures as directed.
• Maintain Medical Records – Keep accurate, up‑to‑date records in compliance with policies and regulations.
• Ensure Safety and Infection Control – Follow all protocols to maintain a safe environment for residents and staff.
• Prevent Spread of Illness – Adhere to proper infection control procedures at all times.
• Engage in Ongoing Education – Maintain certifications and stay current with best practices

Pay Range: $33.11 - $41.59/hr. (This rate is inclusive of the $3.40/hr. Michigan Skilled Nursing Facility Direct Care Worker Pass-Through approved rate; current approval through September 30, 2026).

Shift Premiums: 2:30pm - 10:30pm $1.90 | 10:31pm - 7am $1.35 | Weekend Fri 10:31pm - Sun 11pm $1.00
