Job summary

Transitions At Home is currently hiring a Full-Time Licensed Practical Nurse (LPN) to join our team in the Mount Horeb area. We offer a competitive wage and benefits to eligible employees, with no night shifts or weekends and a focus on compassionate home health care.

As an LPN, you will perform skilled nursing visits, manage medications, and coordinate services in patients' homes under the supervision of the RN Clinical Manager.

Qualifications

• Two (2) years’ nursing experience, 1 year in Home Health (preferred)
• CPR Certification
• A valid Wisconsin Driver's License and Car Insurance

Responsibilities

• Provide skilled nursing care including routine visits, medication management, treatments, and coordinate services
• Counsel patient and family in home per physician orders and plan of care
• Support with on-site care tasks under RN Clinical Manager supervision
