Fundamental in Wisconsin is seeking a Licensed Practical Nurse to join our nursing team for full-time or part-time AM and PM shifts. You will provide compassionate patient care, work under supervision, and collaborate with the care team to deliver quality services.

Qualifications

• Must be a licensed LPN in good standing with the state (as required)
• 1+ years of nursing experience in a long-term care facility strongly preferred
• Reliable transportation required

Responsibilities

• Works under direct supervision using the state-specific Nurse Practice Act, Practice Guidelines and nursing judgment.
• Responsible for delivering nursing care to patients requiring care, collecting patient data, making observations and reporting pertinent information to the care of patients.
• Will also direct and supervise care given by other nursing staff members as needed
