Job summary

Cleveland Care & Rehab in Oklahoma is seeking a Licensed Practical Nurse (LPN) to join our compassionate nursing team. The role covers all aspects of patient care, with a focus on delivering high-quality services to residents and documenting needs accurately.

We offer a 12-hour shift schedule from 6pm to 6am, a $5,000 sign-on bonus for full-time candidates, paid time off after 30 days, and comprehensive benefits including medical, dental, vision, life, and a 401(k) plan.

Qualifications

• Graduate of accredited LPN School of Nursing.
• One year of nursing experience preferred.
• Valid LPN license for Oklahoma.
• Valid CPR certification.
• Working email address and phone number required.
• Ability to work cooperatively as part of a team.
• Effective communication with residents and families.

Responsibilities

• Assist in all aspects of nursing.
• Provide services that meet standards beyond company and regulatory requirements.
• Initiates and leads individualized nursing care plans.
• Assess and document the resident’s condition and needs.
• Implement physicians’ orders accurately and promptly.
• Assign nursing care to team members per resident needs.
• Administer medications.
• Start IVs and perform treatments as assigned.
• Maintain supplies and equipment in a safe, clean manner.
• Other duties as assigned.

Current shift availability-

• 6pm to 6am

  -Thursday through Saturday, every other Wednesday

  -Sunday through Tuesday, every other Wednesday

Essential Functions of Licensed Practical Nurse (LPN):

• Highly motivated and able to assist in all aspect of nursing

• The candidate will provide services that exceed company and regulatory standards

• Initiates and leads individualized nursing care plans

• Assesses and documents the resident’s condition and nursing needs

• Accurately and promptly implements physicians’ orders

• Assigns nursing care to team members in accordance with the resident’s needs and the person’s capabilities and qualifications

• Administers medications

• Starts IVs and performs treatments for assigned residents

• Documents that treatment as required

• Ensures that supplies are utilized economically, and that equipment is clean and maintained a safe manner

• Others duties as assigned

Qualifications of Licensed Practical Nurse (LPN):

• Graduate of accredited LPN School of Nursing

• One year of nursing experience is preferred

• Valid LPN license for Oklahoma is required

• Valid CPR certification

• Candidates must maintain a working email address and phone number for employer communication.

• Ability to work cooperatively as a member of a team

• Ability to communicate effectively with residents and their family members, and at all levels of the organization

• Ability to react decisively and quickly in emergency situations

• Ability to maintain confidentiality

• Knowledge of training techniques for clinical staff
