Job summary

Wesley Halton Housing is seeking an Intensive Case Manager to support intake and day-to-day admin tasks within a client-centered, housing-first program. The role serves Halton Region’s homeless population and involves housing placements, income support, and community linkage.

The position is full-time, temporary (six months) with a 40-hour week, paying $25.11 per hour, and includes shifts that may cover days, evenings, and weekends.

Qualifications

• Post-secondary degree/diploma in Human Services or related field.
• Must qualify to register with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
• Experience working with adults and families who may be experiencing homelessness, mental health and substance use issues, while providing non-judgmental supports that empower.

Responsibilities

• Assist in intake supports and day-to-day administrative services.
• Place individuals and families into permanent leased units and secure housing.
• Maintain regular communication with housing providers and partners to support clients.
