Zusammenfassung

WelbeHealth, LLC. is seeking Home Care Assistants to provide in-home personal care and support to participants in Modesto, CA. You will join an interdisciplinary team, delivering ADLs, instrumental tasks, and safety support on a full-time schedule.

Responsibilities include assisting with daily activities, documenting vital signs, and supporting the rehabilitation plan. Bilingual skills are a plus, with health and safety certifications required or pursued after hire.

Qualifikationen

• Must have or be willing to obtain CPR and First Aid within 180 days of hire.
• Must be registered with Home Care Registry (HCA) prior to start date.
• 1+ years of caregiving experience with elderly or senior patients preferred.

Aufgaben

• Provide hands‑on personal care and assistance with ADLs in participants' homes.
• Assist with Instrumental ADLs such as laundry, light housekeeping, shopping, and meal prep.
• Measure and document vital signs as instructed.
• Act as a medical escort to appointments and assist with safe transfers.
• Support the Rehabilitation team by carrying out home exercise programs as indicated in the care plan.
• Provide translation services as needed.

Kenntnisse

BLS/First Aid

CNA/HHA certification

Bilingual (preferred)
