Job summary

Silverado Hospice seeks an RN On-Call to provide after-hours nursing support, ensuring timely assessments and continuity of care for patients and families. You will perform comprehensive nursing evaluations, update plans of care, and document within 24 hours of visits. Must hold a valid RN license with at least 1 year of nursing experience.

Hospice experience is preferred but not required. Valid driver’s license, reliable transportation, ability to lift 50 pounds, and clearance of background

• Submit accurate documentation within 24 hours of visits.
• Coordinate services across settings to ensure quality of care.
• Follow approved care plans and communicate with physicians as needed.

We're hiring an RN On-Call – a compassionate and skilled nurse who will provide after-hours support, ensuring timely assessments and continuity of care for hospice patients and families. This role is essential to delivering dignified, responsive care during critical moments.

What You'll Be Doing

• Perform comprehensive nursing evaluations and after-hours assessments for patients with changes in condition
• Assess for signs and symptoms of discomfort, including pain, and take prompt, appropriate action
• Initiate and update plans of care as needed; provide nursing follow-up and caregiver instruction
• Submit accurate documentation within 24 hours of visits
• Coordinate services across settings to ensure quality and appropriateness of care
• Follow approved care plans, modify as needed, and communicate with physicians when appropriate

Qualifications

• Must clear a criminal background check, physical, and drug screening

Shift Schedule: Full Time (on- call), Monday - Thursday, 5:00pm - 8:00am

Anticipated pay range: $85,000 USD - $95,000 USD
