Job summary

Suncrest Hospice is seeking a Registered Nurse to provide in-home and facility-based care as part of an interdisciplinary team. You will perform psychosocial assessments, coordinate with social workers and other professionals, and travel across the service area to visit patients in homes, SNFs, and assisted living facilities.

Qualifications

• Current RN licensure in the state(s) of practice.
• Ability to collaborate with nurses, social workers, chaplains, and other team members.
• Willingness to travel across the service area and drive as needed.

Responsibilities

• Provide care for a group of patients visiting homes, SNFs, or assisted living facilities.
• Interdisciplinary team member contributing psychosocial assessments and community resource knowledge.
