Suncrest Hospice is seeking a Registered Nurse to provide compassionate care to patients in homes and facilities. You will be part of an interdisciplinary team in case management, admissions, or resource roles, visiting patients across the service area. Strong teamwork, communication, and time management are essential.

This full-time, salaried position offers competitive pay and benefits, with travel across the service area supported by the organization.

Formación

• RN licensure (in good standing) in the state(s) of practice.
• Able to work within a team and collaborate with nurses, social workers, chaplains, and CNAs.
• Ability to work 1-1 with patients while receiving strong team support.
• Capable of independent day-to-day work using phone, email and meetings.
• Strong time management to schedule visits and meetings and triage urgent needs.
• Able to learn and use an electronic medical records system.
• Willing to travel across the service area with reliable transportation and valid driver’s license.

Responsabilidades

• Provide care and services for a group of patients in homes or facilities.
• Be a contributing member of the interdisciplinary team in case management, admissions or resource roles.
• Coordinate visits, documentation, and patient communications to ensure high-quality hospice care.
