Job summary

Choice Hospice, Inc. is seeking a dedicated Hospice Aide (HHA) to provide compassionate direct patient care in patients’ homes within the Conroe, TX area.

Under RN direction, you will assist with transfers, nutrition, and daily care, while documenting changes to ensure continuity of care. The role emphasizes infection control, respectful communication with patients and families, and the ability to work collaboratively with the care team.

Qualifications

• Current CNA/HHA & LTC certifications
• Valid Driver’s License
• Valid Auto Insurance
• CPR Certification
• HomeCare HomeBase experience preferred

Responsibilities

• Provide direct patient care under RN direction and aid plan of care
• Assist patient with safe transfers and ambulation per policy
• Report changes and document care to ensure continuity of care
• Maintain infection control and provide a clean, safe environment
• Communicate respectfully with patients, families and staff
• Support privacy and respect for patient belongings
• Assist with proper nutrition and fluid intake as needed
• Contribute to efficient operation and time management of the agency
• Perform treatments and procedures per policy
• Support the agency philosophy and policies to ensure quality of care
