Job summary

Devotion Hospice is seeking a dedicated Hospice Aide (HHA) to provide direct patient care under the supervision of an RN in the patient’s home or residence, following the Aid Plan of Care and promoting a clean, safe environment.

This full-time role in Conroe, TX requires current CNA/HHA and LTC certifications, a valid driver’s license and insurance, CPR, and comfort with documentation, transfers, nutrition support, and respectful communication with patients and families.

Qualifications

• Current CNA/HHA & LTC certifications.
• Valid Driver’s License.
• Valid Auto Insurance.
• CPR Certification.
• HomeCare HomeBase experience preferred.

Responsibilities

• Provide direct patient care to patients under direction of the RN and according to the Aid Plan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy.
• Provide necessary skill to appropriately report changes and document pertinent information and care rendered to patients to ensure continuity of care.
• Practice infection control principles and maintain a clean, safe environment.
• Promote respectful communication with patient/family and other staff members.
• Support privacy and protect patient and property.
• Assist with proper nutrition and adequate fluid intake.
• Contribute to efficient agency operations and manage time effectively.
• Perform treatments and procedures according to agency policy.
