Job summary

St. Croix Hospice is seeking a dedicated Hospice Aide (CNA) to join our interdisciplinary care team in patients' homes across Illinois.

Under the supervision of a Registered Nurse, you will provide compassionate, hands-on personal care and support to comfort and dignity at the end of life. The role requires current Illinois CNA certification, a valid driver's license, reliable transportation, and a commitment to 1:1 patient care, documenting observations, and promoting safety and comfort during

Qualifications

• Current Illinois CNA certification.
• Valid driver’s license, clean driving record, and auto insurance.
• Provide personal care including bathing, skin/nail care, oral hygiene, dressing/undressing.
• Record vital signs and report observations to the care team.
• Assist with meal planning, preparation, and feeding as needed.
• Promote patient comfort, dignity, and safety.
• Travel to patient homes throughout the day and provide care as scheduled.
• Maintain accurate documentation of services provided.

Responsibilities
