St. Croix Hospice is seeking a compassionate Hospice Aide (CNA) to join our interdisciplinary team in home-based patient care.

You will provide hands-on support under RN supervision, helping with personal care and daily activities to promote comfort and quality of life for patients at the end of life. The role requires an active CNA license in Indiana, a valid driver’s license, and the ability to travel to patient homes.

Qualificações

• Current CNA certification in the state of practice (Indiana).
• Valid driver’s license, clean driving record, and auto insurance.
• Provide personal care including bathing, grooming, oral hygiene, dressing and undressing.
• Record vital signs and report observations to the care team.

Responsabilidades

• Assist with personal care and activities of daily living in patients’ homes.
• Record vital signs and report observations to the care team on schedule.
• Assist with meal planning, preparation, and feeding as needed.
• Promote comfort, dignity, and safety for patients receiving end-of-life care.
• Travel to patient homes throughout the day and provide hands-on care as scheduled.
