Job summary

Aveanna Healthcare is seeking a full-time Physical Therapy Assistant (PTA) to deliver in-home therapy services under physician guidance. You will assess, teach, and implement interventions, coordinating with an interdisciplinary team to support optimal patient outcomes.

Responsibilities include direct therapy per plan of care, patient education, and thorough documentation. Strong communication and organizational skills are essential to success in this home health setting.

Qualifications

• Graduation from a Physical Therapist Assistant education program from an accredited university/program.
• Current, unrestricted state license as a Physical Therapy Assistant in the state of practice
• Proficient in clinical assessments, documentation and compliance with policies and procedures

Responsibilities

• Provides direct physical therapy according to the plan of care.
• Communicates with patient/caregiver about the plan of care.
• Provides skilled therapy services and treatments for patient care and safety.
• Records observations and progress notes to ensure continuity of care.
• Prepares discharge instructions and communicates discharge plan with patients and caregivers.
• Participates in in-service programs and ensures confidentiality of patient records.
