Job summary

Aveanna is seeking a Physical Therapy Assistant (PTA) for full-time work in home health. The PTA will deliver and teach therapy interventions under the physician-directed plan of care, coordinating with an interdisciplinary team to support patient well-being in Sarasota and Charlotte Counties.

The role requires Florida PTA licensure, CPR, and strong organizational and communication skills. Experience in home health and OASIS documentation is preferred, with a focus on patient safety and timely

Qualifications

• Graduation from an accredited PTA program.
• Current, unrestricted Florida PTA license and CPR.
• Home health experience preferred.
• Strong organization and communication skills.
• Proven ability to work with interdisciplinary teams.

Responsibilities

• Provide direct physical therapy under supervision per plan of care.
• Update patients/caregivers on the care plan.
• Deliver skilled therapy and treatments for patient safety.
• Document patient progress and maintain confidentiality.
• Collaborate with interdisciplinary team; participate in discharge planning.
• Attend in-service programs.
