Job summary

Aveanna Healthcare is seeking a Physical Therapy Assistant to join our home health team in Wisconsin. This full-time role involves providing skilled PT services under the supervision of a PT and per the physician's plan of care.

The position requires a PTA degree, an unrestricted state license, CPR, and a valid driver's license. You will educate patients and caregivers, document progress, and work with an interdisciplinary team to optimize patient outcomes.

Qualifications

• Graduate of PTA program from an accredited university.
• Current, unrestricted PTA license in state of practice.
• CPR certification and valid driver’s license.

Responsibilities

• Provides direct physical therapy according to directions of physical therapist and physician’s plan of care.
• Communicates with patient and caregiver about the plan of care.
• Reports observations to supervisor or physician as appropriate.
• Educates patients and caregivers in use of prosthetic, orthotic and assistive devices.
• Discharges planning and communicates discharge plan with patient and family members.
