Job summary

Windsor Home Health is seeking a dedicated Physical Therapist Assistant to join our home health team in Ohio. You will provide therapy under supervision of a licensed PT, helping patients improve strength, function, and independence in the comfort of their homes.

You will document progress, adapt plans with the PT, and communicate with patients and families to ensure quality outcomes. Travel to patients' homes within a defined area is part of the role.

Qualifications

• Provide physical therapy treatments under the supervision and direction of a licensed Physical Therapist (PT).
• Assist patients with exercises, therapeutic activities, and mobility strategies to improve strength, function and independence.
• Track and document patient progress, adjusting care plans with the PT as needed.
• Communicate with patients, families and healthcare teams to ensure optimal care outcomes.
• Travel to patients' homes within a defined area.
