Responsibilities

Work Environment

This full-time position requires working onsite in the home setting, providing care directly to patients in their living environments. You'll work Monday to Friday with a 40-hour shift, allowing you to develop a rapport with your patients in the comfort of their own homes. Key Responsibilities

• Conduct thorough evaluations and planning for individualized treatment programs for patients referred by their physicians.
• Implement a range of therapeutic interventions and techniques tailored to a variety of specialties, including orthopedic, pediatric, neurologic, and sports medicine.
• Deliver outstanding customer service and engage with patients and their families to foster a positive therapeutic environment.
• Collaborate with fellow healthcare professionals to maximize patient outcomes using evidence-based care practices.
• Administer skilled therapeutic interventions at the patient’s home, consistently maintaining adherence to the physician’s outlined plan of care.
• Educate and empower patients and their caregivers through clear communication and guidance on therapeutic practices.
• Meet established utilization and performance goals to contribute to the overall success of our healthcare facility.

Patient Population / Client Focus

Your primary focus will be on working with diverse patient groups, inclusive of individuals recovering from injuries, managing chronic conditions, and those requiring support in mobility and daily activities. Tailoring your treatments to each patient’s unique circumstances will be essential, ensuring they receive the individualized care and encouragement necessary for their rehabilitation journey.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program (Doctorate of Physical Therapy is preferred).
• Must hold an active State License in Physical Therapy.
• Current certifications in Basic Life Support (BLS) and CPR.
• Strong interpersonal communication skills, both written and verbal, allowing you to interact effectively with patients, families, and team members.
• Proficiency in word processing and computer skills to manage patient records and treatment documentation.

Preferred Experience

• Minimum of two years of experience as a licensed physical therapist, particularly in a home care or outpatient setting.
• Experience with various patient demographics, especially in orthopedic and neurological specialties.
• Demonstrated ability to establish and maintain strong relationships with patients for optimal therapy outcomes.

Compensation and Benefits
