Job summary

Valley Health System is seeking a licensed Physical Therapist to provide direct patient care in the home environment as part of a multidisciplinary team. You will evaluate patients, design individualized plans of care, and supervise assistants while using an electronic record for documentation.

Orientation begins with a 4–6 week precepted period. New hires may receive relocation assistance, sign-on bonuses up to $15k/$25k (based on experience), or up to $30k in student loan reimbursement

Qualifications

• Degree in Physical Therapy required (Bachelor's, Master's or Doctorate).
• CI credentialing course or equivalent required.
• BLS certification (Healthcare Provider) - AHA required.

Responsibilities

• Provide direct, one-on-one physical therapy in patients' homes.
• Supervise PT aides and collaborate with nursing, OT, speech and social work teams.
• Document assessments and treatments in the electronic health record via laptop.
• Participate in orientation, policy review, and continuous professional development.
• Engage with a multidisciplinary team to tailor plans of care.

Department HOME HEALTH - 207151 Worker Sub Type Regular Work Shift First Shift (United States of America) Pay Range $38.02 - $57.04 Job Description New hires may be eligible for relocation assistance. The Home Health Physical Therapist provides direct patient care in the patient's home environment as part of a multidisciplinary team including nursing, physical therapy, occupational therapy, speech, social work and home health techs. The Physical Therapist evaluates patients and use professional judgment to design the individualized plan of care which best meets the patient's needs. The Physical Therapist provides direct, one-on-one patient care, and will also supervise care provided by assistants. The Home Health agency relies heavily on technology; email is a primary means of communication, and the therapist will use a laptop to complete documentation in an electronic record. The patient population varies widely across the spectrums of acuity, age, and diagnosis. The Physical Therapist works under the direction of a clinical team manager with support from a rehab clinical coordinator.

Employment begins with a 4–6-week orientation with a seasoned preceptor to ensure that the clinician is well prepared when they begin treating on their own. Additional job duties include performance improvement opportunities, policy and procedure review and development, research, etc. Working in home health provides endless variety of setting, scenery, and clinical challenges, and provides a unique opportunity to enter and work within the real-life environments of our patients, bringing meaningful change to their daily lives.

Education Bachelors, master's or doctoral degree in Physical Therapy is required. A completed CI credentialing course, home study adult learning course, or equivalent, is required.

Experience Minimum two years of clinical experience (4,160 hours) is required.

Certification & Licensure Physical Therapy licensed or eligible in the state of assignment (Virginia or West Virginia) is required. Licensure in a secondary state may be required depending on geographical area assigned. BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved is required. New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.

Valid driver's license and reliable transportation is required.

Qualifications Basic computer and keyboarding skills or participation in computer training within the first year of employment. Competent to successfully interact with patients, according to their special needs which may apply based upon the age of the patient served. Must have access to transfer electronic data. Must not have been convicted of or have pending charges of any barrier crimes.
