Job summary

WelbeHealth in Pasadena, CA is seeking a Home Care Assistant to support participants in their homes as part of the IDT. You will provide hands-on personal care, assist with daily living activities, and participate in care planning with the IDT to ensure safety and comfort.

You will document vitals, assist with home exercise programs, and serve as a medical escort when needed. We offer comprehensive benefits, PTO, holidays, a 401K match, and bonus potential.

Qualifications

• High school diploma or equivalent is required.
• Current BLS/First Aid certification required.
• Bilingual abilities preferred.

Responsibilities

• Provide hands-on personal care and assistance with ADLs in participants' homes.
• Assist with instrumental ADLs such as laundry, light housekeeping, shopping and meals.
• Document vital signs and participate in care planning with the IDT.
• Transport participants to medical appointments as needed.
• Support rehabilitation home exercise programs as trained.
