Job summary

WelbeHealth is seeking Home Care Assistants to provide hands-on personal care and support to participants in their homes as part of an interdisciplinary team. You will assist with daily living activities, monitor health, and contribute to care planning.

Qualifications

• High school diploma or equivalency.
• Current BLS/First Aid Certification required.
• CNA or HHA certification preferred.
• Bilingual preferred.
• Must be registered with Home Care Registry prior to start date.
• CPR and First Aid certification within 180 days of hire.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living (ADLs) in participants' homes.
• Assist with Instrumental ADLs such as laundry, light housekeeping, shopping and meal prep.
• Measure and document participants' vital signs as instructed.
• Act as a medical escort to appointments and assist transfers.
• Support Rehabilitation team with home exercise programs per care plan.
• Provide translation services as needed.
