Job summary

WelbeHealth is hiring Home Care Assistants to support seniors in their homes as part of an interdisciplinary team. You will deliver hands-on personal care, assist with ADLs, and contribute to care planning under supervision.

Responsibilities include caregiving tasks, vital signs, and assisting with home exercise programs. BLS/First Aid and CNA/HHA certification are valued, with optional bilingual capabilities.

Qualifications

• High school diploma or equivalency.
• Current BLS/First Aid Certification required.
• Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred.
• Bilingual preferred (Mandarin, Cantonese, or Spanish highly preferred).
• Must be registered with Home Care Registry (HCA) prior to start date.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.
• 1+ years of caregiving experience with elderly or senior patients highly preferred.

Responsibilities

• Provide hands‑on personal care and assistance with activities of daily living (ADLs) to participants in their homes, including bathing, dressing, grooming, toileting, and transferring.
• Assist participants with Instrumental ADLs, including laundry, light housekeeping, shopping, and light meal preparation.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort on vehicles to medical appointments, supporting participants with safe transfer in and out of their homes.
• Assist the Rehabilitation team by carrying out home exercise programs as indicated in the care plan developed by the IDT, after sufficient training as needed.
• Provide translation services as needed.
