Job summary

WelbeHealth is seeking Home Care Assistants to help seniors stay in their homes, coordinating with the IDT to deliver safe, personal care. The role includes visiting multiple participants daily, providing ADL support, and contributing to care plans.

Qualifications

• High school diploma or equivalency.
• Current BLS/First Aid certification required.
• CNA or Home Health Aide (HHA) certification preferred.
• Bilingual preferred.
• Must be registered with Home Care Registry prior to start date.
• CPR and First Aid certification within 180 days of hire and keep current; hire preferred.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living (ADLs) in participants' homes.
• Assist with Instrumental ADLs like laundry, light housekeeping, shopping, and meal prep.
• Measure and document vital signs as instructed.
• Act as medical escort to appointments and support safe transfers.
• Assist Rehabilitation team with home exercise programs per care plan.
