Job summary

WelbeHealth is hiring Home Care Assistants in Pasadena to support seniors at home within a comprehensive care model. You will join the interdisciplinary team, delivering personal care and helping implement care plans.

Responsibilities include ADLs, instrumental tasks, vital signs, and assisting with rehab exercises. A supportive work environment offers medical insurance, PTO, a 401K match, and additional benefits.

Qualifications

• High school diploma or equivalent is required or preferred.
• BLS/First Aid certification is required at start.
• CNA or Home Health Aide certification strongly preferred.
• Bilingual abilities are advantageous for patient care.

Responsibilities

• Provide hands-on personal care and assistance with ADLs in participants' homes.
• Assist with instrumental ADLs such as laundry, light housekeeping, shopping, and meal prep.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort, assisting with safe transfer to and from appointments.
• Support Rehabilitation team by carrying out home exercise programs per care plan.
• Provide translation services as needed.
