Job summary

WelbeHealth in Los Angeles seeks Home Care Assistants to deliver in-home care to participants as part of an interdisciplinary team. You will provide hands-on personal care and assist with ADLs while coordinating with the IDT to support care plans and safety.

The role offers a stable full-time schedule with 8-hour days, 2-5 visits per day, travel time reimbursement, and a comprehensive benefits package including medical, PTO, holidays, a 401(k) match, and opportunities for growth.

Qualifications

• High school diploma or equivalency.
• Current BLS/First Aid Certification required.
• CNA or HHA certification preferred.
• Bilingual preferred.
• Must be registered with Home Care Registry prior to start date.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living (ADLs) to participants in their homes.
• Assist participants with Instrumental ADLs, including laundry, light housekeeping, shopping, and light meal preparation.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort on vehicles to medical appointments, supporting safe transfers.
• Assist the Rehabilitation team by carrying out home exercise programs as indicated in the care plan.
• Provide translation services as needed.
