Qualifications

Knowledge of the DSM and related symptoms.
Proven advocacy and case management abilities.
Strong written, interpersonal, and verbal communication skills.
Sensitive to diverse populations and maintains confidentiality.

Responsibilities

Provide case management, psycho education, crisis intervention, advocacy, and community liaison services.
Complete documentation timely (shift logs, incident reports, service notes).
Monitor and educate about medications to support self-dosing.
Teach ADLs and community skills in individual and group settings.
Assist with client transportation between residences, office, and community sites.
Support staff with scheduling client appointments and coordinating with treatment teams.
