Qualifications

• Experience or education in advocacy, case management, and effective communication.

Responsibilities

• Provide case management, psychoeducation, crisis intervention, advocacy, and community liaison services.
• Accurately document shift logs, incident reports, and service notes in a timely manner.
• Monitor medications and educate clients to support self-dosing where appropriate.
• Teach community skills including ADLs in individual and group settings.
• Assist Rehabilitation Counselors with transportation of clients between residences, office, and community locations.
• Complete required training hours (20 per year) and adhere to safety rules and facility policies.
