Qualifications

• Education: High School diploma or G.E.D and graduated from an approved LPN/LVN program.
• Experience: Two years of relevant LVN/LPN experience in an acute care setting.
• Licenses/Certifications: Current LVN/LPN license in the practicing state; BLS & ACLS from the American Heart Association; Pedagogy IV Certification within 30 days of hire.

Responsibilities

• Provides safe, compassionate nursing care to patients as directed by the RN and the patient’s plan of care.
• Performs duties that promote quality, safe patient care and excellent service.
• Completes accurate and timely documentation reflecting the plan of care.
• Conducts focused patient assessments per nurse practice act and policies.
• Administers medications per organizational policies.
• Reports data to identify problems and support goals/outcomes in collaboration with the care team.
• Communicates with patients and families for planning and coordination of care.
• Maintains a clean and safe work environment and adheres to infection control procedures.
