Qualifications

• Bachelor’s degree in social science or related field required.
• Certified Peer Counselor credential preferred for FHARPS program.
• Successful background check required and valid First Aid/CPR.
• Ability to work independently and as part of a team.

Responsibilities

• Serve as Primary Case Manager for assigned caseload and coordinate client needs.
• Coordinate referrals and develop individualized plans.
• Collaborate with care team to ensure access to treatment and appointments.
• Provide crisis intervention and community outreach as needed.
• Maintain confidentiality and accurate documentation in EMR.
