Job summary

Community House Mental Health Agency in Seattle, WA seeks a FHARPS Residential Case Manager to provide treatment and care coordination in a residential setting within the diversion program. You will work with a care team to coordinate services, document progress in EMR, and support residents toward treatment goals.

The role emphasizes trauma-informed care, strong collaboration, and community-based support, with a focus on safety, empowerment, and accessible services.

Qualifications

• Apply for Agency Affiliated Counselor credential with Washington DoH within 30 days of hire.
• Pass criminal background check before hire date.
• Obtain/maintain First Aid/CPR within 30 days of hire.
• Hold a valid driver license with clean driving record.
• Legible handwriting and ability to do simple math; navigate EMR charts.

Responsibilities

• Serve as secondary Case Manager and primary on-site contact for residents at assigned location.
• Coordinate with internal/external departments for referrals and timely intake.
• Collaborate with care team to ensure access to treatment and appointments, including transportation.
• Ensure all interactions reflect trauma-informed care principles (safety, choice, trust, collaboration, empowerment).
• Coordinate access to medications following the seven rights and proper documentation.
• Complete daily EMR documentation for resident interactions and medications dispensed.
• Participate in shift-change meetings and required documentation.
• Support on-site cleaning and activities to promote skills training and independence.
