Job summary

COMMUNITY HOUSE MENTAL HEALTH AGENCY in Seattle, WA is seeking a FHARPS Case Manager to provide primary case management and care coordination for adults with mental health needs. This role covers outreach, documentation, crisis support, and collaboration with a multidisciplinary team.

The position emphasizes trauma-informed practice, transportation coordination, and timely service plan updates. Competitive hourly pay and full-time schedule with two shift options are offered.

Qualifications

  • Bachelor’s degree in social science or related field.
  • FHARPS program: credentialing as Agency Affiliated Counselor is acceptable and preferred.
  • Apply for Agency Affiliated Counselor credential with Washington Department of Health within 30 days of hire, if not licensed.
  • Successfully pass criminal background check before hire date.
  • Obtain/maintain First Aid/CPR card within 30 days of hire.
  • Hold a valid driver's license with clean record.

Responsibilities

  • Serve as Primary Case Manager for assigned caseload, advocating for and coordinating client needs and services; ensure Service Plans and Crisis Plans are completed on time.
  • Coordinate with internal and external departments to coordinate client referrals and timely intake; develop individualized plans.
  • Collaborate with care team to ensure access to treatment, support, and appointments, including transportation support.
  • Outreach to clients in homes or community settings to assess living situation and stability; provide interventions as needed.
  • Ensure all interactions follow trauma-informed care principles: safety, choice, trust, collaboration, and empowerment.
  • Coordinate access to medications through trained staff, following seven rights of medication use and proper documentation.
  • Complete daily EMR documentation for patient interactions, medications, and other required entries at least twice monthly.
  • Practice good financial stewardship of resources; monitor time, supplies, and asset usage.
  • Meet key performance indicators; participate in discharge planning and care summaries as needed.
  • Provide after-hours crisis intervention when assigned, including crisis assessments and coordination with involved professionals.

Education

Bachelor’s degree in social science or related field
Agency Affiliated Counselor credential (FHARPS acceptable)
Credentialing: Agency Affiliated Counselor within 30 days
