Lifeworks, Inc. is seeking a dedicated Case Manager to support adults with ID/DD in a nonprofit setting. Join a collaborative team and help individuals achieve employment, education, and social goals through advocacy and comprehensive case management.

The role emphasizes direct support, ISP development, and documentation in electronic and physical records, with a schedule that includes Monday-Thursday days and Friday mornings.

Qualifications

• Experience working with Individuals with ID/DD.
• A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services.
• Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks.
• Valid Driver's License required.
• Valid MAP certification is preferred.
• Excellent verbal and written communication skills.

Responsabilités

• Assists individuals served to obtain and maintain employment aligned with goals.
• Collaborates with the team to advocate for and support individuals; attends meetings as needed.
• Supports self-employment, education, social growth, health wellness, and community mobility.
• Provides case management support using electronic (Icentrix) and paper records.
• Writes and implements Individual Support Plans (ISP) and documents accordingly.
• Prepares monthly and weekly progress notes; maintains ISP communications.
• Offers on/off-site Life Enhancement Curriculum support for employment, social and health training.
• Leads community volunteer and learning experiences; completes payroll/billing daily.
• Performs other duties as needed at different locations.
