Job summary

Lifeworks, Inc. in Massachusetts is hiring a full-time Case Manager to support adults with ID/DD. You will partner with individuals to promote engagement, independence, and meaningful community involvement.

We seek an enthusiastic team member with strong verbal and written communication skills, a valid driver’s license, and the ability to pass fingerprint-based background checks. Lifeworks provides competitive pay and an excellent benefits package in a rewarding, supportive environment.

Qualifications

• Experience with individuals with ID/DD required.
• Must pass fingerprint background checks and have a valid driver’s license.
• MAP certification preferred.

Shift:Monday-Thursday 8am - 4pm, Friday 7:30am - 3:30pm

Lifeworks is hiring a full time Case Manager to work with Individuals with ID/DD. We are a nonprofit human service agency that provides adults with developmental disabilities the opportunity to live engaged and meaningful lives. We're searching for an enthusiastic individual who will bring their personality, an independent work ethic, and a strong belief in enhancing the lives of those we serve .

Requirements:

• Experience working with Individuals with ID/DD
• A Bachelor's degree or an associate degree in a related field with two years of experience, or HS diploma and five years of related work experience in Human Services.
• Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
• Valid Driver's License required
• Valid MAP certification is preferred
• Excellent verbal and written communication skills
