Lifeworks, Inc. in Massachusetts is seeking a full-time Case Manager to support adults with developmental disabilities. You will work with individuals and groups on self-care, daily living, communication and social skills, while coordinating services with an interdisciplinary team and families.

Qualifications

• Experience with individuals with ID/DD is required.
• Excellent verbal and written communication skills are essential.
• Must have a valid driver’s license and pass background checks.

Responsibilities

• Provide individual and group instruction in self-care, daily living, and communication.
• Provide direct care for ADLs and assist with transfers as needed.
• Coordinate services with an Interdisciplinary Team and families.
• Document progress notes, write summaries, and maintain participant records.
• Participate in supervision sessions and team meetings.
